Application form can be obtained (A) from the University office on payment of Rs 500/- cash D. D (for superspecialty programmes Rs 1000/-) (B) Through a requisition letter with name of course, opted for, detailed correspondence address with contact no. and e-mail address, along with a demand draft of Rs 600/- (for superspecialty programme Rs 1100/-) drawn on any nationalized bank, in favour of “Register, Dr D Y Patil Vidyapeeth” payable at Pune OR (C) By downloading from the University Website. (The downloaded form shall be submitted along with a demand draft covering Form Fee + Test Fee + Postage Rs. 100/-)

Fees should not be sent by Money Order. The University shall not be responsible for any postal delay and loss. Late Fee for all courses shall be Rs 500/-.
